We earn 5% for office & classroom supplies on all your purchases.

Forms to be Collected at Final Registration
Emergency Information Form   -Required for each student
Authorization to Administer Medication   -Must be renewed each year if your child needs to take medicine during the school day
Severe Allergy Form   -If your child has severe allergies
Background Check Release Form   -If you want to volunteer in the classroom
Volunteer Driver Agreement   -If you want to drive on field trips
Safe Environment Training Sessions   -If you have never attended, you must complete before you volunteer in the classroom
September 2010 Lunch Order Form   -Orders are due Tuesday, August 17 at Final Registration

2010 Immunization Requirements
 The new requirements for immunizations are as follows:
All Kindergarteners and First graders must have 2 varicella (Chicken Pox) vaccines.
All 6th and 7th graders need to have a TDAP booster.

 If you visit your child’s physician this summer, please remember to pick up a copy of your child’s most recent vaccination records.  If you are fortunate enough not to have to visit your physician this summer, please call and have them send or fax a copy to the school office at 303-762-0156.

Mornings beginning the week of August 9 & regular school hours beginning August 16

Mark your calendars: Our 61st Annual Jamboree & Auction will be held on Sunday, October 10, 2010. Please help us to collect auction donations...print the attached donation request letter and give copies to businesses in your area.

Gym Maintenance Weekend is now Saturday and Sunday, August 14-15. Please call Mary Menardi at 303-639-9035 if you would like to help!
