Field Trip Authorization Form - Print, complete, sign and return to the school office.

Authorization to Administer Medication - To be signed by physician.

Background Check Inquiry Release - Must be completed prior to any volunteer service.

Volunteer Driver Agreement - Must be completed by parents wishing to drive for field trips.

Grocery Certificate Order Form - St Louis School receives 5% for each grocery certificate you purchase through us.  Place your order in the office and we’ll return it to you via backpack mail the next day! Print this form, complete the order information, and return it with payment to the school office.

SCRIP Order Form - SCRIP offers non-profit organizations the opportunity to raise funds without asking anyone to spend a penny more than they already do.  Numerous retailers offer gift cards and certificates (SCRIP) which are accepted the same as cash, and in return, our school keeps a percentage of SCRIP sales. SCRIP makes a great gift! We'll be placing orders November 1, November 15 & December 1 for Christmas delivery.  Processed orders will be shipped to the school in about 3 days. Orders will resume in mid-January after Christmas break. Return your order form and payment to the school office. Your SCRIP will be sent home with your child when it arrives! Checks should be made payable to St. Louis Catholic School.
